Hi there. I would like to ask you if you could look at Help:Creating a new entry. That will explain the format that we use here. Furthermore, add importScript('User:Conrad.Irwin/creation.js'); to your monobook.js to enable the acceleration script, which will allow you to accelerate the creation of forms of words. The script isn't really a script at all. All it does it turn certain links in templates green when the word doesn't exist and the acceleration script can automagically make the entry for you.
